I currently accept Blue Cross/Blue Shield, Aetna, Penn State Student Resources, and UPMC insurance plans. It’s possible that your insurance card may have one of these carrier names and/or logos on it, but that does not mean that counseling will be covered. Therefore, please check with your insurance provider to make sure that I am in-network prior to attending your first appointment.

If you plan to use out-of-network benefits, I can provide you with forms to submit for reimbursement.

If you plan to pay out-of-pocket, the initial appointment costs $160 and each subsequent session costs $140. I accept cash, credit, or check payments in person or through my secure client portal. Payments are collected at the conclusion of each session. I will file all insurance claims under “Brad Carmichael, MS.Ed, LPC, LLC.” If you require any further documentation for your insurance (e.g., statements, receipts, etc.), please ask and I will do my best to help!

There is no charge for cancellations made more than 24 hours before your appointment. If you do not notify me within this time, then I reserve the right to collect a cancellation fee of up to 100% of the service fee.
